For new clients:

1. Call the Kaiser Mental Health Line at 707-571-3778

• Request a referral through Carelon Behavioral Health (formerly Beacon Health Options) to a non-Kaiser provider.

• You can let them know you'd like a referral due to a desire to have more frequent appointments than Kaiser can offer, to see a specific therapist, and/or to have in-person appointments

2. Kaiser will take about 2 weeks to send you an AUTHORIZATION FOR MEDICAL CARE BY A NON-KAISER PERMANENTE PROVIDER

3. After you receive your authorization number from Kaiser (usually begins with 314 ...), call Carelon Behavioral Health at 855-267-1511

• Request a BILLING AUTHORIZATION NUMBER (usually begins with 01- ...)

• You will need to provide your therapist's full name and service address (1622 4th Street, Santa Rosa, CA 95404)

4. Once you have all the information above, contact your therapist at Kendall Individual, Family and Marriage Therapists, Inc. / Fourth Street Christian Therapists, 707-919-3218. Be prepared with:

• Your Kaiser member number

• Your BILLING AUTHORIZATION NUMBER from Carelon Behavioral Health and the effective dates of authorization
