  • Treatment, Payment, or Health Care Operations: Your information may be used for treatment, payment, or health care operations without requiring your written authorization. This includes consultations with other healthcare providers to enhance the quality of care.

  • Lawsuits and Disputes: In legal situations, we may disclose health information in response to court orders or lawful processes.

Certain Uses and Disclosures Requiring Your Authorization

Specific uses and disclosures of psychotherapy notes require your authorization, except in defined circumstances. These include situations like treatment, training, legal defense, compliance investigations, and threats to health and safety.

Certain Uses and Disclosures Without Your Authorization

We may use and disclose your health information without your authorization for reasons such as complying with state or federal law, public health activities, health oversight activities, judicial and administrative proceedings, law enforcement purposes, and more.

  • Accessing and Correcting Your Information: You have the right to access and get copies of your information. If you believe there is an error, you can request corrections.

  • Getting a List of Disclosures: Request a list of instances where we disclosed your information, and we will provide this information within 60 days.

  • Monday – Friday: 8 AM – 9 PM
    Saturday appointments may be available through some clinicians.

  • Pricing: 50-minute sessions range from $110 to $200 depending on the training, experience, and availability of the clinician.

    We also offer opportunities for FREE Couples or Family THERAPY in the community through our trainee program in which we provide supervision for graduate students in counseling psychology as they complete the requirements for clinical hours in their degree programs. These student clinicians are serving as volunteers in local 501 C3 settings for which we at Fourth Street Christian Therapists provide supervision.
